('Duuo') for the purposes of creating and managing my Duuo account, confirming my identity, reviewing my insurance needs and determining the suitability of the insurance products offered by Duuo, providing me with quotes, assessing my applications for insurance, issuing insurance policies, and administering my insurance policies, including assessing claims, detecting and preventing fraud, and performing business and statistical analyses. I further consent to the disclosure of my personal information by Duuo to the insurers that underwrite my insurance policies ('Insurers'), and to the collection, use and disclosure by such Insurers of my personal information for the purposes described in the previous sentence. I understand that my personal information may be disclosed to the service providers of Duuo and the Insurers, that such service providers are located both within and outside of Canada, and that my personal information may be subject to disclosure to those duly authorized under the laws of Canada and applicable foreign jurisdictions. If I provide Duuo with the personal information of any other person(s), I hereby certify that I have the express approval of such person(s) to provide such information to Duuo and the Insurers and to consent, on their behalf, to the collection, use and disclosure of their personal information as described above, which I hereby give.
